Sports Physicals

Sports Physicals will be held at the Junior High School at the end of October, 2007

All students who wish to try out for a sport (basketball, baseball, softball) must have a current physical – less than 13 months old - on file in the Nurse’s Office in order to tryout. This physical may be by your own physician or by the school physician. There is no charge for the school sports physical. If you choose a school sports physical you must complete a Pre-participation Health History Form. This form is available in the Nurse’s Office and on this web site and must be returned to the school nurse no later than October 30, 2007. Please contact the school nurse with any questions or concerns you may have.
