Student School Bus Transportation Confirmation Form
We are requesting your cooperation in providing information to help us accurately assess the transportation needs of the district. Families who do not intend to use school bus transportation next year are still asked to complete the form to ensure the district has complete and accurate data for planning purposes. Students who are graduating from Case High School this year do not need to complete the form.
We are asking that families electronically submit this form by Monday, June 2. If a form is not received for your student, school bus transportation may not be assigned for the 2025-2026 school year. We kindly ask that you complete a separate form for each student currently enrolled in Swansea Public Schools. Paper copies of this form are available upon request at your student’s school. Thank you for your attention to this important matter.
